Enthält: Ausfertigung, Pergament, im ehemaligen Umbug drei Einschnitte für Siegelstreifen. Das Blatt ist später - wohl bei neuer Verwendung - etwas tiefer umgebogen worden. Am oberen und am linken Rand. zu letzterem parallel oben und unten auch durch den linken Teil des Textes Nadelstiche, ein Loch im Pergament ist verflochten mit einem schmalen aus Federkiel geschnittenen Streifen, der auch durch Bohrlöcher in einem kleinen untergelegten herzförmigen Bleiplättchen gezogen ist. Das Blatt hat als Umschlag gedient, es ist stark verschmutzt, die Schrift ist abgegriffen. Rückschrift (16. Jh.) "Quondam Hans Gnotzman to Olrickstorpe nunc xlvj Johan Tr[ude] to sunte Margreten. - Johan Trude xix marck. xix ß rente up paschen ad lumina". Auf dem ehemaligen Umbug. aber zu der Schrift des Textes der Urkunde auf dem Kopf stehend (wohl noch 16. Jh.): "j daler to kalen, noch iiij m alle frigdage j ß den armen noch x ß alle stille frigldage] …", unter dem Umbug weitere Notizen, die zu dem Inhalt der Urkunde in keiner Beziehung stehen. Auf der Rückseite Aufschrift (17. Jh.?), wohl für das ehemals eingeheftete Register: "Memorien gudt. V.", am linken Rand der Vorderseite: "Memorien geldt 4593 mr. 8 ß. Calandes geldt hovetstol 1757 mr. Anno 1599".
